Guides - Create new form

You can create a new empty form on the overview of forms. This overview can be accessed under the menu item 'Settings' > 'Forms'.

At the top of the page you can see a list of all forms. You have the following options:

• Form name: You can see and change the name of the form.
• Edit form: You can edit the form, e.g. enter the spaces that the customer should fill out. See Edit form for more information.
• Edit PDF report: You can make a PDF report that can be created with the customer's answer. Here you can, e.g., determine which of the customer's answers should be included in the report. See Edit PDF report for more information.
• Export data: You can export all customers' answers from a particular period to a .csv file that can be read by e.g. Microsoft Excel. See Export data for more information.
• Active: You can determine whether the form should be active. If a form is inactive, it cannot be filled out by customers.
• Delete. By ticking the box and clicking on 'Update', you can delete the form.

Remember to click on 'Update' to save any changes!

At the bottom of the page you can create a new form. In order to create a new form, enter the name of the form. You then need to choose whether to create a new empty form, or a copy of an existing form. Finally, you must determine which type of form you want to create. You can choose between creating a simple form where the customer is asked to fill out a form after having booked a particular service, or a three-part form (questionnaire/result/satisfaction) that can be filled out before, during, and after a treatment. When you have ticked the box opposite the desired type, click on ‘Create form'. The form has now been created!
