What are forms?

With GECKO Booking's Form Add-On you can create questionnaires and forms that you and/or your customers can fill out. You can use the add-on to obtain information about the customer before a treatment, to fill out during a treatment, or to obtain information about the customer after a treatment.

The form add-on allows you to create two different types of forms:

  • Simple form: The customer is asked to fill out this type of form after having booked a service to which the form is linked. A particular type of the simple form is the satisfaction survey that the customer is e-mailed after the service has been completed (see Satisfaction survey for more information).
  • Three-part form (questionnaire/result/satisfaction): This type of form consists of three parts that must be filled out at three separate times: 1) Before the appointment, the customer is asked to fill out a form when (s)he books a service to which the form is linked. This part of the form can be used to obtain additional background information about the customer. 2) During the appointment, you can fill out a form that can be used as a result form in which you enter the results of the treatment. This could be used if the treatment is an examination and you want to save the results in GECKO Booking. 3) Finally, the customer is asked to fill out the final part of the form after the appointment has been completed. The customer will be asked to fill out the form the next time (s)he logs in to GECKO Booking after the service has been performed. The three-part form can be linked to one or multiple services. Depending on your needs, you can use the same form for all of your services or link the form to one particular service only.

Please note! Only the simple form is included in the Form Add-On by default. If you want to create three-part forms, this feature must be activated by GECKO Booking.


When a new form has been created (see Create new form for more information), you need to enter the spaces the customer should answer (see Edit form for more information). The form then needs to be linked to one or multiple services. When a form is linked to a service, the customer is asked to fill out the form when booking the given service. A simple form is linked to one or multiple services under the settings of the form (see Edit simple form settings for more information), while the three-part form is linked to a service under the settings of the service (see Link form to service for more information).

You can view a customer's answers on the customer's profile (see Customer forms for more information), and you can export all customers' answers to e.g. Microsoft Excel (see Export data for more information). Furthermore, you can make a PDF report (see Edit PDF report for more information) with the customer's answers, which can be used internally in the company or be e-mailed to the customer (see Edit form answers for more information).
