Edit form

When you have created a new form, it is created as an empty form containing no spaces. You can create the spaces that the customer should fill out by clicking on 'Edit form' opposite the desired form on the list of forms.

At the bottom of the page you can create a new space. In order to create a new space, enter the name of the space that functions as the "question" the customer should answer. If the form consists of three parts, you then need to determine the position of the space. You can choose between before booking (questionnaire), where the customer will be able to fill out the space at the time of booking, during booking (result form) where you can fill out the space during the treatment itself, and at next log in (satisfaction form), where the customer is asked to fill out the space the next time (s)he logs into online booking after the treatment has been performed. You then need to choose the type of the space. You have the following options:

  • Drop-down list (select): The customer must answer by choosing between predetermined options. The options are shown in a drop-down list. You can determine the amount and name of the options. When this type has been chosen, a tiny space appears in which you can write the number of options. The name of the options can be entered when you edit the space (see Edit form space for more information).
  • Approve / accept (checkbox): The customer must approve/accept the 'question' by ticking of a box. This space type is required, meaning that the customer must tick off the box in order to continue.
  • Text only (no spaces): This type of space is different from the rest in that it is not a space the customer should fill out but a 'space' in which you can write a text. This can be used to insert headlines or remarks in the form. For instance, you can insert a remark at the bottom of the form named ‘Thanks for your time'.
  • Tick (checkbox): The customer must answer by either ticking or not ticking a box. The space type is not required, meaning that the customer can choose whether or not to tick off the box.
  • Tick answer (radio-buttons): The customer must answer by choosing between predetermined options. The customer must choose between the options by ticking the box opposite the desired answer. You determine the amount and name of the options. When this type has been chosen, a tiny space appears in which you can enter the number of options. The name of the options can be entered when you edit the space (see Edit form space for more information).
  • Number space – small (text): The customer must answer by entering a number. No other characters than numbers can be written in the space.
  • Number space – small with decimals (text): The customer must answer by entering a number. Numbers, decimal points, and decimal commas can be written in the space.
  • Text space – small (text): The customer must answer by writing a text. All types of characters can be entered in the space. The customer can write one line only.
  • Text space - large (textarea): The customer must answer by writing a text. All types of characters can be entered in the space. The customer can write multiple lines.

Finally, click on 'Create space'. The space has now been created and appears in the list of questions/spaces.


Please note! When you have created a form space, you must change the settings of the space so that they match your needs. For instance, you must enter the name of the options the customer can choose between. This is done by clicking on ‘Edit' opposite the desired space in the overview (see Edit form space for more information).


At the top of the page you can see a list of all spaces. You have the following options:

  • ID: You can see the ID of the space.
  • Ques.: You can see the substitution code for the space. If you insert this substitution code in the PDF report, the code will be substituted with the space (see Edit report content for more information).
  • Answer: You can see the substitution code for the answer to the space. If you insert this substitution code in the PDF report, the code will be substituted with the customer's answer (see Edit report content for more information).
  • Answer num.: You can see the substitution code for the numerical value of the answer. When you insert this substitution code in the PDF report, the code will be substituted with the numerical value of the customer's answer (see Edit report content for more information).
  • Name / question: You can see the name of the space the customer should answer. In order to change the name of the space, press ‘Edit' (see Edit form space for more information).
  • Type: You can see the type of the space.
  • Position: If the form consists of three-parts, you can see the position of the space.
  • Page: You can determine which page the space should appear on. If the form consists of many spaces, it can be preferable to divide it into multiple pages.
  • Priority: You can determine the priority of the space. The priorities of the spaces determine the order in which the spaces appear; the higher the priority of a space, the lower on the list it is shown. Which numbers are chosen as the priorities are not important as the system only cares about the priorities' relationship to each other.
  • Edit: You can edit the space, e.g. enter the name of the options between which the customer can choose. See Edit form space for more information.
  • Delete: You can delete the space.

Remember to click on ‘Update' to save any changes!
