Link form to service
In order to enable customers to fill out the form, each form must be linked to one or multiple services. The customer will be asked to fill out the form, when (s)he books a service to which the form is linked. Simple forms are linked to services under the settings of the form (see Edit simple form settings for more information). Three-part forms are linked to a service under the settings of the service (‘Settings' > ‘Services' > ‘Edit settings'). |

Please note! You can determine when the first part of the three-part form should appear, i.e. the part that the customer is asked to fill out at the time of booking. You can determine whether the form should be shown before the customer chooses the date and time of the service or after the customer chooses the date and time of the service. This is done under the advanced settings (‘Settings' > ‘Advanced settings' > ‘Front-end' > ‘Forms'). |
